Nodulo Tiroideo

Articolo del 10/07/2025

The thyroid, though small in size, plays a vital role in overall wellbeing. It regulates metabolism, heart rate, body temperature, and is essential for the development and function of the nervous system.

One of the most common conditions affecting the thyroid is the presence of thyroid nodules—solid or fluid-filled lumps that form within the gland.
While the majority of thyroid nodules are benign, they should never be ignored. Some can impair thyroid function or cause local compressive symptoms.

When should a thyroid nodule be evaluated?
It's important to assess a thyroid nodule if it:

  • Grows rapidly over time

  • Causes difficulty swallowing or breathing

  • Is associated with voice changes (e.g. persistent hoarseness)

  • Occurs alongside symptoms of thyroid dysfunction such as fatigue, unexplained weight loss, anxiety, constipation, cold intolerance, or a racing heart

What tests are essential for evaluating a thyroid nodule?
A precise, personalized diagnostic pathway is key to determining whether a nodule requires monitoring or removal. This includes:

🔹 Thyroid Ultrasound
Offers a detailed view of the gland, helping to assess the presence, size, and characteristics of nodules, as well as any signs of current or past inflammation.

🔹 Blood Tests
Measure thyroid function (TSH, FT3, FT4) and detect inflammatory or autoimmune conditions (thyroid antibodies).

🔹 Thyroid Scintigraphy
Evaluates nodule activity. A "hot" nodule absorbs the contrast agent and produces hormones, while a "cold" nodule does not—rarely, this may suggest malignancy.

🔹 Fine-Needle Aspiration (FNA) Biopsy
A crucial test for determining the nature of the nodule. Cytological analysis of the sample reveals whether it's benign or suspicious/malignant.

When should a thyroid nodule be surgically removed?
Surgical removal is advised if:

  • The nodule is confirmed or suspected to be malignant

  • It causes significant compressive symptoms (e.g. difficulty breathing or swallowing)

  • It continues to grow, impacting the patient's quality of life

  • It produces excessive thyroid hormones (hyperthyroidism) and does not respond to medication or relapses after treatment suspension
